Purchase $25 of ExquisiCat brand cat litter products from PetSmart stores between June 20, 2011 and August 21, 2011 and receive a $25 PetSmart® Gift card. Purchases must total $25 before sales tax. To claim your Gift Card, complete this mail-in offer form, and mail this completed offer form, along with your original PetSmart receipt(s) with your qualifying ExquisiCat cat litter purchases circled ("Proof of Purchase"), to: PetSmart ExquisiCat Rebate Offer: P.O.
